Welcome to the Kindergarten online registration process, which begins on February 9, 2024. Scituate Public Schools offers Full-Day Kindergarten for children who are 5 years old on or before September 1, 2024.

Prepare for the Kindergarten Registration Process

Developmental History Form

The Developmental History Form is required for students entering Kindergarten. The form is a writable PDF that offers you the option of saving it to your computer, where you can complete, save, and then upload it when prompted while completing the online registration. 

If your child currently attends the Early Childhood Center program, you only need to complete the Developmental History Form.

Required Registration Documentation For All New Students

Gather digital copies, either scanned or photographed, of required documents. During the online registration process, you will be prompted to upload the required document files below:

  1. Developmental History Form
  2. Copy of Birth Certificate
  3. Copy of Current Immunization Record and recent physical, including Lead levels.
  4. Proof of Residency
  5. Proof of Occupancy
  6. Proof of Parent Identification

Online Registration

Once you have gathered digital copies, either scanned or photographed, of the required documents, you will be ready to register online. During the process, you will be prompted to upload the required document files.
